  • This form must be filled out even if you have attended sessions at our clinic before.

• $300 DEPOSIT AND $50 NONREFUNDABLE PROCESSING FEE

  • After completing the registration form, check your email for a message from Social Thinking Stevens Creek inviting you to sign into your Client Portal

    • Please allow 2-3 business days for this email to arrive

  • You can submit payment for the deposit and processing fee through the Client Portal or you can call 408-244-2005 to pay by credit card

  • We cannot process your application without the deposit & processing fee

  • Please send this link https://forms.gle/JPkXgXsnF3oWEcQH6 to as many of your child’s teachers as possible. This form is available as a PDF upon request.

• RECENT PICTURE OF YOUR CHILD

• A SHORT VIDEO OF YOUR CHILD (2-3 MINUTES)

  • This might include your child:

    • Pretend playing with someone else (e.g., parent, sibling, friend, etc.)

    • Having a conversation with someone about a recent event or experience (e.g., day at school, trip to the beach, etc.)

• MOST RECENT REPORTS

  • Please send any of the following (if applicable):

    • Individualized Education Program (IEP)

    • 504 Plan

    • Psycho-educational assessment

    • Neuropsychological evaluation

    • Speech and language assessment/report

    • Doctor’s letter OR report including medical diagnosis (not required but helpful if seeking insurance reimbursement)
